What You Will Need to Register:

-  Your team assignment code from your coach

-  A username and password for the parent/guardian: If you are a first time parent registering your player with UYSA, you will create this during the online process. If you forget your username or password after it is created, you will find a FORGOT USERNAME/PASSWORD link during step 2

-  Email Address

-  Proof of Age: First time or former rec players will need to upload one of the following: 

• Birth certificate, current driver's license, passport, Alien Registration Card issued by the U.S. Government (USCIC form I-551), Uniformed Services Identification and Privilege Card (DD Form 1173), or a document issued by a legitimate government agency wherein birth date is verified as part or the document issuance process

• Digital Face ONLY photo: MUST be FACE ONLY. No shoulders or chest, no hats, no sunglasses, no funny faces.
